Transitional Living Program

This program of supervised apartment living promotes:

bulletthe development of daily living skills
bulletsocialization and recreational skills
bulletpre-vocational and vocational skills
bulletand provides support during the challenging times

Community Outreach Program

This program provides:

bulleton-going socialization/recreational opportunities
bulleton-going support
bulletcrisis intervention for mental health clients

Services are provided through two program components:

This is a space for:

bulletmental health clients to "drop-in" and hang out
bulletfor groups, activities, play, socialization, and a safe place to be

Benefits of the Transitional Living Program:

Intensive support and training through:

Outcome:

Daily and community living skill development

¨        Routine daily monitoring provides opportunities for early intervention as problems arise which in turn reduces recidivism

¨        Improved medication compliance resulting in reduction of signs & symptoms of mental illness

¨        Improved personal self-care resulting in increased self-esteem and overall health

¨        Increased likelihood of smooth community reintegration and success

¨        Improved budget/money management skills for more self-sufficient community living

¨        Increased self-sufficiency which results in improved self-esteem

Socialization and Recreational skill development

¨        Improved and healthy options are developed to help clients maintain a quality social life resulting in reduced isolation – a leading causes of recidivism for mental health clients

¨        Healthy alternatives are learned to reduce the likelihood of substance abuse as a coping mechanism for isolation and loneliness

Pre-vocational/Vocational skill development

¨        Improved likelihood that if capable, the client can secure and maintain employment

¨        If employment is not an option, skills are developed to help clients become productive community contributors as volunteers

Self-esteem, Communication, and Relationship skill development

 

¨        Improved self-esteem and communication skills help the clients develop and maintain healthier relationships that

§          helps to reduce the signs and symptoms of mental illness

§          provides support when the mental illness becomes a problem

Advocacy

¨        Reduces stigma

¨        Provides needed support that might otherwise not be available

¨        Teaches clients to advocate for themselves

Supervised Apartments

¨        A safe place to live while clients prepare to reintegrate to the community

Music Wellness

¨        Stress reduction; Illness Prevention & Early Intervention

¨        Development of a healthy form of self-expression, anger management, and coping

Drop-In Services

 

¨        Basic needs can be met through easy access to phones, office equipment such as copier, fax machine, computer, etc.

¨        Staff provides early intervention and support as needed which reduces recidivism

¨        Clients feel a sense of consistency in support services as they need them

¨        Clients feel a sense of belonging and “home”

Advocacy

¨        Reduces stigma

¨        Helps clients access decreasing mental health services

¨        Provides needed support that might otherwise not be available

¨        Teaches clients to advocate for themselves
