Hope for Recovery from Mental Illness

Vision & Mission Statements:

·    Our Vision is to be a model program that provides community based mental health services to mental health clients in transition to community living.

·    Our Mission is to provide low cost, high quality transitional housing and supportive care to those persons with mental illness, enabling them to gain increased independence and self-reliance.  The organization will seek to prevent emotional and economic dependency as well as to promote personal self sufficiency and self support.

Staff & Board:
·    Our Board:  Transition House, Inc., is a private non-profit 501(c)3 agency which started in April of 1982.  The agency’s governing Board of Directors, comprised of community volunteers, is dedicated to supporting mental health clients as they make the transition to community living with an emphasis on high quality of service with low cost for service delivery.
·    Our Staff includes 5 dedicated employees whose years experience at Transition House, Inc., total over 66 years, plus student volunteers from OU as appropriate.

Services to Adults with Serious Mental Illness through:
·    Transitional Living Program
o      In FY’07 served 29 clients 
o      Clients Receive:
o      Individualized Recovery Plans
§  Weekly meetings with a Recovery Coordinator
§  Weekly meetings with the Clinical Director
§  Daily groups and activities
§  Housing - up to three clients can live in one of four apartments while in the program
·    Drop-In Center
o      The Drop-In Center provides space for Transition House, Inc. groups, activities and socialization.
o      Center Highlights:
§  Located at 700 Asp, Suite 4
§  Available for use by clients week days from 9:00 am to 5:00 pm
§  Transitional Living Program clients may use the Center after hours with appropriate approval
§  The Center is used for groups, activities, and provides a safe place to be
§  Furnished with cable TV/DVD, bathroom, and other items that make it a comfortable space for clients
·    In FY ‘07, a total of 99 persons in recovery from mental illness were served through Transition House, Inc.

·    In FY ‘07, the overall cost = $46 per unit of service
o      Transitional Living Program cost = $58/transitional living day
o      Outreach cost = $26/contact
